Beth Israel Lahey Health Launches Hospital at Home Program at Beverly and Addison Gilbert Hospitals
Program Delivers Inpatient-Level Care for Patients at Home
Beverly and Gloucester, Mass. – Beth Israel Lahey Health (BILH) has launched its Hospital at Home program at Beverly and Addison Gilbert Hospitals, providing inpatient-level care for patients in the comfort of their homes. BILH first introduced this program at Lahey Hospital & Medical Center (LHMC) in Burlington this summer and plans to offer this service at other hospitals within the BILH system over the next two years.
Patients in the program have 24/7 access to their care team and are seen with a combination of in-person and telehealth visits. Patients are provided with all necessary technology including equipment to monitor their vital signs, a telephone to connect directly with their care team and a computer tablet for video visits with the care team. Typical services like lab tests, IV therapies, X-rays and ultrasounds are all available to be performed in the home.
“Hospital at Home allows our patients to recover in the comfort of their homes, with the same access to care that they would receive in an inpatient setting,” said Michael Rowan, Executive Vice President for Hospital and Ambulatory Services, BILH. “By giving more patients access to this model of care, we are helping to increase care in the community.”
“Hospital at home programs have been shown to reduce readmission rates and recovery time for patients, while providing high-quality care in their homes,” said Sue Stempek, Vice President of BILH Hospital at Home. “We have already seen success with our program at Lahey Hospital & Medical Center, and we are excited to launch this service at Beverly and Addison Gilbert Hospitals.”
“This new care model will give our patients at Beverly and Addison Gilbert Hospitals the opportunity to recover in their homes, while staying connected seamlessly to their care teams,” said Tom Sands, President of Beverly and Addison Gilbert Hospitals. “Hospital at home programs are helping hospitals address capacity concerns, while also increasing access to community-based care.”
The BILH Hospital at Home care team consists of physicians, advanced practice providers, registered nurses and other health care professionals who treat and monitor patients in their own homes with individualized care plans similar to how a patient would receive care while admitted to the traditional hospital.
Care teams work directly with patients to determine their eligibility for Hospital at Home.
Since its launch in August, Hospital at Home at LHMC has served more than 65 patients to date. Learn more about the Hospital at Home program.
