From Route 114 East or West
From Route 114 East or West, turn onto Route 1 North. Follow Route 1 North to the exit for Route 62 West toward Middleton. At the end of the ramp, turn right onto Route 62/Maple Street. The main outpatient entrance, called the North Entrance, leads to the first floor.
The South Entrance, located at the rear of the building, leads to the second floor. Once inside, a greeter will assist you.
