Access to your hospital records

Your Medical Records

You can access your medical records online any time through our patient portal. Your patient portal account includes test results and other details about your medical care.

We provide patients with copies of their records via email, fax, USPS, in-person or via My Lahey Chart.

To request copies of your medical record, please fill out the Authorization to Release Medical Records form. The authorization form must be completed, signed and dated.

Please note: we do not accept typed or electronic signatures on this form.

How to Request Copies of Medical Records
  1. Call the Health Information Management Department at 978-712-1470 to request copies of your patient medical records.
  2. Or, visit us in-person at 133 Brimbal Avenue, Suite A in Beverly MA. Please call us first. When you come to the office, you must present your ID, submit payment and sign and date an Authorization to Release Medical Records form. 

The office is open Monday through Friday from 8:30 am - 5:00 pm.

All patients requesting records must complete an authorization to release form. This form must be (hand) signed and dated. Note: We do not accept typed or electronic signatures.

How to Send Copies of Medical Records  to Another Facility

If you need a copy of your medical records sent to another medical facility or for personal reasons, you must make a written request. This allows you to authorize us to release your records. 

Charges may apply for copies of medical records. If there is a fee, we will bill you or contact you in advance. 

Write us a letter or download and complete this Authorization to Release Medical Records form.

Birth Certificates

If you or your child was born at Beverly Hospital, you can request a copy of the birth certificate. To request a copy, use the National Center for Health Statistics to find out who to contact for your vital records.

While Beverly Hospital collects information included on birth certificates, the certificates are town records and are not available through the hospital.
