Beverly and Addison Gilbert Hospitals Launch Programs to Improve Care Integration
Innovative projects from Phase 2 of Massachusetts Health Policy Commission CHART Grant will better coordinate patient care and integrate behavioral health

BEVERLY, MA – December 4, 2015 – On Thursday, Dec. 3, Beverly and Addison Gilbert Hospitals, affiliates of Lahey Health, commemorated the launch of innovative new programs designed to integrate technology, increase coordination and manage complex care across a patient’s lifespan. The hospitals were awarded grants to fund these programs in Phase 2 of the Health Policy Commission’s (HPC) Community Hospital Acceleration, Revitalization, and Transformation (CHART) Investment Program. The CHART Phase 2 grants are two-year investments intended to accelerate the transformation of hospitals by focusing on outcome-oriented aims, including reducing waste, improving safety, maximizing appropriate usage, and enhancing behavioral health care.
“Beverly and Addison Gilbert Hospitals have developed truly innovative ways to not only improve the health of the communities they serve, but also reduce the overall cost of health care in the Commonwealth,” said Howard Grant, JD, MD, president and chief executive officer of Lahey Health. “The CHART 2 programs at Beverly and Addison Gilbert Hospitals align with Lahey Health’s strategic vision of increasing patient satisfaction by responsibly managing the cost of care without sacrificing quality, safety, or clinical excellence.”
The Hospitals’ CHART 2 programs are:
Addison Gilbert Hospital was awarded $1.269 million to scale up the activities of its High-Risk Intervention team. This team’s goal is to improve quality, build efficiencies and reduce costs by addressing gaps in the care of patients with complex social, behavioral and medical needs in the community. The team aims to reduce 30-day readmissions for patients with a history of recurrent hospital or emergency department use, social complexity, and/or need for palliative care services through the use of new procedures and workflows, new relationships within the hospital and community partners, and through the collection and analysis of data.
Beverly Hospital received $2.5 million to transform the quality, safety and cost of care delivered in the hospital and in the community in collaboration with community-based partners. The hospital will work toward addressing the Institute for Healthcare Improvement’s” Triple Aim,” incorporating next-generation health technology and enhancing behavioral health for high-risk patients. Beverly Hospital’s goal is to implement an operational plan to reduce unnecessary acute care utilization through evidence-based care for high-risk patients. Beverly’s plan will increase cost efficiency, enhance patient access to social services, improve clinical interventions, and optimize coordination to provide patient care in the most appropriate setting in order to effectively engage patients and their families.
