Patient Experience

Surgery at Lahey Outpatient Center, Danvers has been recognized as
a 2014 Guardian of Excellence Award winner by Press Ganey
Associates, Inc for outstanding patient experience. This award
recognizes top-performing facilities that consistently sustain
performance in the top 5% of Press Ganey clients over the course of
one year.

Echocardiography Services Earn Accreditation
We are pleased to report that Addison Gilbert Hospital, Beverly
Hospital and Beverly Hospital at Danvers have once again been
granted a three-year accreditation in adult transthoracic
echocardiography by the Intersocietal Accreditation Committee
(IAC). This highly regarded seal of approval is granted only to
facilities that demonstrate to a panel of experts that quality
patient care is being delivered in compliance with national
standards. Echocardiography is an essential tool when diagnosing
disorders of the heart and blood vessels. Accurate diagnoses are
dependent on the skill of the sonographer performing an exam, the
quality of equipment used, the knowledge and skill of physicians
interpreting the results, and the quality assurance measures in
place. The IAC's comprehensive accreditation application process
includes detailed case study review.

Beverly and Addison Gilbert hospitals were recently selected as
the Gold and Silver winners, respectively of the 2011
Gatehouse Media's Reader's Choice, "Favorite Hospital - North of
Boston". Reader's Choice affords residents of the North
Shore to share their favorite local places, services and
businesses. Nominations take place between January and
February and the winners were announced on June 23.

Beverly Hospital and Addison Gilbert Hospital Among
Thomson Reuters 100 Top Hospitals for 2011
Beverly and Addison Gilbert hospitals have been named among
Thomson Reuters 100 Top Hospitals in the U.S. We are
one of only two community hospitals in Massachusetts and the only
hospital on the North Shore to be named to this year's list. The
Thomson Reuters 100 Top Hospitals® study is annual, quantitative
research that identifies the hospitals with the best facility-wide
performance. At the heart of this research is the 100 Top
Hospitals National Balanced Scorecard, a set of measures that
evaluate performance excellence in clinical care, patient
perception of care, operational efficiency, and financial
stability.

Addison Gilbert Hospital, Beverly Hospital and Beverly Hospital
at Danvers Receive Echocardiography Accreditation
Addison Gilbert Hospital, Beverly Hospital and Beverly Hospital
at Danvers recently received accreditation of their
echocardiography laboratories from the Intersocietal Commission for
the Accreditation of Echocardiography Laboratories (ICAEL).
Accreditation status signifies that the facilities have been
reviewed by an independent agency which recognizes the laboratory's
commitment to quality testing for the diagnosis of heart
disease.
The ICAEL certification process was established with the support
of the American Society of Echocardiography, the American College
of Cardiology and the Society of Pediatric Echocardiography to
provide a peer review mechanism to encourage and recognize the
provision of quality echocardiographic diagnostic evaluations by a
process of voluntary accreditation. A non-profit organization, the
ICAEL is dedicated to ensuring high-quality patient care and to
promoting healthcare.

Ledgewood Rehabilitation and Skilled Nursing Center Receives
Distinguished National Quality Award
Ledgewood Rehabilitation and Skilled Nursing Center, Northeast
Senior Health's 123-bed short-term rehabilitation and long-term
care facility located on Beverly Hospital's upper campus, was
recently named a recipient of the 2010 Silver Quality Award by the
American Health Care Association (AHCA). The AHCA National Quality
Award Program is based on the core values and criteria of the
Malcolm Baldrige National Quality Award Program, the nation's
premier recognition for quality achievement.
AHCA Quality Awards include an entry level Bronze Commitment to
Quality Award; a more rigorous Silver Achievement in Quality; and
the Gold Excellence in Quality Award which requires applicants to
address the Baldrige Program's Health Care Criteria for Performance
Excellence in its entirety. Recognized with the Bronze Award in
2007, Ledgewood's Silver Award this year demonstrates their ongoing
commitment to providing the highest caliber care to patients and
residents.

Beverly Hospital Endoscopy Suite Recognized for Quality and
Safety by The American Society for Gastrointestinal Endoscopy
Beverly Hospital's Endoscopy Unit has been recognized for its
quality and safety practices. Beverly Hospital's unit is the only
endoscopy unit in Massachusetts to receive this honor.

Beverly and BayRidge Hospitals Achieve NICHE Designation
Beverly Hospital and BayRidge Hospital have achieved Nurses
Improving Care for Healthsystem Elders (NICHE) designation. NICHE
is a national initiative based at the Hartford Institute for
Geriatric Nursing at New York University designed to facilitate the
adoption of evidence-based geriatric best practices throughout
institutions to improve nursing care for older adult patients. With
this designation, the hospitals have joined an elite group of
organizations who participate in this program. There are only 10
other NICHE sites in Massachusetts, 260 in the United States, and
301 worldwide. This designation is an extension of the NICHE
designation achieved by Addison Gilbert Hospital in 2007 and
reflects Northeast Hospital Corporation's commitment to provide
quality care to our geriatric inpatients, who represent more than
60 percent of our admissions.

Wound and Hyperbaric Medicine Center Achieves Award of
Distinction
Beverly Hospital's Wound and Hyperbaric Medicine Center has been
named as a Center of Distinction by Diversified Clinical Services
(DCS), the wound care management company with whom we partner to
provide exceptional patient care. This award recognizes the
Center's success in meeting or exceeding DCS's company benchmarks
for 12 consecutive months in healing outcomes, patient
satisfaction, outlier management and days to heal. In fact, the
benchmark for wounds healed is 79.5 percent and we achieved 97
percent; the benchmark for days to heal is 47.4 days and we
achieved 31 days; and the benchmark for patient satisfaction is 91
percent and we achieved 97 percent. DCS partners with approximately
300 wound care centers across the country, and ours is one of only
ten centers to achieve this distinction.

Northeast Hospital Corporation Recognized for Consistently High
Quality
Northeast Hospital Corporation (NHC) has been identified by the
Quality Indicator Project® (QI Project) as an organization with
consistently high levels of performance across all clinical quality
measure sets for more than a one-year period of time. The QI
Project was initiated in 1985 by the Maryland Hospital Association
and now includes healthcare organizations across the country and
around the world. A study conducted by the QI Project concluded
that high-performing organizations such as NHC are differentiated
from other organizations by three key factors:
- The ability to concurrently collect, analyze and manage data on
patient care practices, which is evident in the ongoing work of our
performance improvement (PI) teams and committees;
- Physician engagement and participation in the review and
analysis of data and in the development of action plans, which our
physicians exemplify by leading 10 of our 16 PI teams and by their
participation in evidence-based practice data review and
action-planning processes;
- Support and engagement of senior executives, which our
administrative, physician and governing board senior management
team demonstrates by identifying organizational goals geared toward
improving quality of care and through their active participation on
our Patient Care Assessment Committee.

excellence in commitment to the community, teamwork, and increased
patient satisfaction scores.

Northeast Hospitals Earn Quality Respiratory Care Recognition
in National Program
Beverly and Addison Gilbert hospitals have earned Quality
Respiratory Care Recognition (QRCR) under a national program aimed
at helping patients and families make informed decisions about the
quality of the respiratory care services available in
hospitals. About 700 hospitals or approximately 15% of
hospitals in the United States have applied for and received this
award.
The QRCR program was started by the American Association for
Respiratory Care (AARC) in 2003 to help consumers identify those
facilities using qualified respiratory therapists to provide
respiratory care. Hospitals earning the QRCR designation ensure
patient safety by agreeing to adhere to a strict set of criteria
governing their respiratory care services.
To qualify for the recognition, Beverly and Addison Gilbert
hospitals provided documentation showing they meet the following
conditions:
- All respiratory therapists employed by the hospitals to deliver
bedside respiratory care services are either legally recognized by
the state as competent to provide respiratory care services or hold
the CRT or RRT credential;
- Respiratory therapists are available 24 hours;
- Other personnel qualified to perform specific respiratory
procedures and the amount of supervision required for personnel to
carry out specific procedures must be designated in writing;
- A doctor of medicine or osteopathy is designated as medical
director of respiratory care services.
The AARC's QRCR program grew out of growing concerns among
health care leaders and the general public regarding the safety and
quality of health care services provided to patients. Hospitals
that meet the QRCR requirements provide a level of respiratory care
consistent with national standards and guidelines, and should be
commended for their commitment to quality care. A list of QRCR
hospitals is maintained at the association's website for consumers,
YourLungHealth.org.
Respiratory therapists are specially trained health care
professionals who work under physician's orders to provide a wide
range of breathing treatments and other services to people with
asthma, chronic obstructive pulmonary disease, cystic fibrosis,
lung cancer, AIDS, and other lung or lung-related conditions. They
also care for premature infants and are key members of lifesaving
response teams charged with handling medical emergencies.
The AARC is a membership organization representing more than
46,000 health professionals involved in respiratory care
nationwide.

Leading Healthcare Institute Names Beverly Hospital Among 100
Top Hospitals in Nation for Fifth Year
Fifth National Recognition for Beverly Hospital in Seven Years
for Excellence in Patient Care.
In a recent study released by Thomson, a leading national
healthcare institute, Beverly Hospital was named one of the
nation's 100 Top Hospitals® for the fifth year. This recognition
also marks the fifth time in seven years that Beverly Hospital has
been nationally recognized for excellence in patient care.
Receiving the 100 Top Hospitals distinction, not once but five
times is a major achievement for Beverly Hospital. The
hospital was rated against more than 2,800 acute general hospitals
in a year-long statistical analysis based solely on objective,
quantitative performance standards that are consistent across all
U.S. hospitals. Beverly Hospital achieved 100 top status in
large part because of exceeding the median performance of large
community hospitals benchmark peer group in five of the nine
categories.
The categories measured include:
- Patient Outcomes: Survival rates for all patients, plus
low rates of surgical complications like infections and severe
bleeding after surgery.
- Patient Safety: Avoiding unwanted events, like
respiratory failure, infections and unexpected deaths in low-risk
diseases, which occur infrequently, but which all hospitals are
working to reduce.
- Core Measures: A set of basic care practices that all heart
attack, heart failure and pneumonia patients should receive.
- Efficiency: Length of patient stay and average expense.
In the best hospitals, all parts of the organization work together
to do the right thing at the right time. When this happens,
patients return to their daily lives faster and costs are usually
lower.
- Financial Stability: A high-performing hospital must be
well-managed financially so it can attract the best healthcare
professionals, acquire new technology and expand services to
improve results for patients. A high-performing hospital must
have enough cash to pay down their bills and must generate a
surplus to be financially stable and invest in the future.
- Growth in Service: Growth from one year to the next in the
number of patients seeking treatment in the hospital's inpatient,
emergency department, ambulatory surgery and outpatient
services. This growth is an indirect reflection of how
satisfied patients are with the services provided. Beverly Hospital
exceeded the median level in this category by more than 80
percent.

Joint Commission on Accreditation of Healthcare
Organizations
By demonstrating compliance with the Joint Commission on
Accreditation of Healthcare Organization's national standards for
healthcare quality and safety, Northeast Hospitals (Beverly,
BayRidge and Addison Gilbert hospitals and the Beverly Hospital
Hunt Center) has earned the Joint Commission's Gold Seal of
Approval. Northeast Hospitals achieved greater than 95 percent
compliance with national standards.

Breast Imaging Center of Excellence Designation
Beverly Hospital at Danvers, Medical and Day Surgery Center has
been designated a Breast Imaging Center of Excellence by the
American College of Radiology (ACR). By awarding
facilities the status of a Breast Imaging Center of Excellence, the
ACR recognizes breast imaging centers that have earned
accreditation in all of the College's voluntary, breast-imaging
accreditation programs and modules, in addition to the mandatory
Mammography Accreditation Program. The breast imaging
services at Beverly Hospital at Danvers are fully accredited in
mammography, stereotactic breast biopsy, breast ultrasound and
ultrasound-guided breast biopsy. Peer-review evaluations,
conducted in each breast imaging modality by board-certified
physicians and medical physicists who are experts in the field,
have determined that Beverly Hospital at Danvers has achieved high
practice standards in image quality, personnel qualifications,
facility equipment, quality control procedures, and quality
assurance programs.
